HCA Florida JFK Hospital is seeking a skilled Clinical Rehab Specialist to join our team. As a Clinical Rehab Specialist, you will play a vital role in identifying candidates for inpatient rehabilitation and ensuring timely, appropriate patient transfers.

Key Responsibilities:
  • Collaborate with the Rehab Medical Director to ensure appropriate patient placement through clinical assessments and screening.
  • Establish relationships with case managers, physicians, payors, and other referral sources to facilitate patient care.
  • Document evidence of rehab admission criteria within established timeframe.
  • Perform on-site and off-site clinical evaluations to assist Rehab Medical Director in establishing patient eligibility for admission.
  • Provide information to patient/family/caregiver related to inpatient rehab and program features.
  • Communicate expectations and requirements for participation in the rehab program and conduct or coordinate tours of the rehab unit.
  • Obtain information relating to patient resources and healthcare insurance benefits.
