Job Summary

We are seeking a highly skilled Primary Care Nurse Practitioner to join our team at ConvenientMD. As a key member of our Advanced Primary Care (APC) model, you will play a vital role in delivering high-quality, patient-centered care to our community.

Key Responsibilities
  • Provide comprehensive primary care services to patients, including health assessments, diagnosis, and treatment plans.
  • Work collaboratively with a multidisciplinary team, including clinical pharmacists, nurses, health coaches, and behavioral health specialists, to deliver holistic care to patients.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Conduct regular health screenings and provide preventive care services to promote healthy lifestyles and disease prevention.
  • Stay up-to-date with the latest medical research, guidelines, and best practices to ensure the highest level of care for our patients.
Requirements
  • Master's degree in Nursing or related field and certification as a Nurse Practitioner.
  • Minimum 2 years of experience in primary care or related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
  • Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments.
