Care Coordinator for High-Risk Patients

A talented and compassionate Care Coordinator is needed to join our team at TotalMed in Indianapolis, Indiana. In this rewarding role, you will provide specialized care for patients transitioning from critical care units, ensuring optimal outcomes in the Step Down Unit setting.
Responsibilities
• Provide comprehensive nursing care to patients transitioning from intensive care to general medical-surgical units, working collaboratively with interdisciplinary teams to achieve positive patient outcomes.
• Monitor and assess patients' vital signs, cardiac rhythms, and respiratory status, utilizing your expertise to make informed decisions and ensure quality patient care.
• Administer medications, perform procedures, and communicate effectively with healthcare team members to facilitate patient progress and optimize outcomes in the step-down unit.
• Educate patients and families on post-intensive care recovery, medication management, and self-care techniques, promoting independence and empowering them to manage their health effectively.
Requirements
• Current RN license
• BLS (Other Certifications as required by facility)
• Two years of recent Step Down experience
• Strong communication and adaptability skills
