Position Overview

CentraCare Health System is seeking a casual Advanced Practice Provider (APP) specializing in Family Medicine. This role involves working autonomously with patients at our clinics, primarily focusing on a diverse patient demographic.

Key Responsibilities

The selected provider will manage patients across all age groups, addressing both acute and chronic health concerns. Responsibilities include:

  • Diagnosing and treating various short-term illnesses.
  • Providing preventive care such as routine checkups, health-risk assessments, immunizations, and screenings.
  • Interpreting lab results and determining appropriate treatment plans.
  • Offering personalized health education and lifestyle counseling.
  • Coordinating care for patients with chronic conditions.

Available Services

Providers will have seamless access to a range of essential services, including:

  • Laboratory services
  • Imaging facilities
  • Diabetes education programs
  • Therapy services (Physical, Occupational, and Speech)
  • Care coordination support
  • Pharmacy services

Work Schedule

  • Availability for daytime clinic hours from Monday to Friday.
  • Optional Saturday morning shifts.
  • No evening, holiday, or on-call duties required.

Qualifications

To qualify for this position, candidates must meet the following criteria:

  • Completion of an accredited nurse practitioner or physician assistant program.
  • Certification as a Nurse Practitioner or Physician Assistant.
  • Current or pending licensure in Minnesota, with the ability to treat patients from pediatrics to geriatrics.
  • Basic Life Support (BLS) certification.

Experience Requirements

Applicants should possess a minimum of two years of relevant provider experience in one of the following fields:

  • Family Medicine
  • Emergency Medicine
  • Urgent Care or Walk-In Clinic settings
