This is an exciting opportunity for a Nurse Practitioner to join our team as a Care Specialist. As a key member of our Senior Community Care (SCC) product, you will be working with patients in various settings, including nursing homes, assisted living facilities, and senior housing. Your primary responsibility will be to provide comprehensive assessments and document findings in a concise and compliant manner.
Key Responsibilities:
- Perform Comprehensive Assessments: Conduct thorough evaluations of patients to identify their medical needs and develop plans to address those needs.
- Document Findings: Accurately record all patient information, diagnoses, and treatment plans in compliance with CMS regulations.
- Communicate Effectively: Collaborate with healthcare professionals, patients, and families to ensure seamless care coordination.
- Travel Between Care Sites: Visit patients at various locations, including nursing homes and assisted living facilities, to provide high-quality care.
Requirements:
- Certified Nurse Practitioner: Possess current certification through a national board and a graduate degree in Nursing (MSN).
- Active APC License: Hold an active and unrestricted license in the State of Connecticut.
- Current DEA Licensure: Have current active DEA licensure or ability to obtain post-hire, per state regulations.
- Access to Reliable Transportation: Possess a reliable vehicle to travel to client and/or patient sites within a designated area.
- Availability to Work: Be available to work 10 hours per week.
