- Complete admissions, social services evaluations, provide for social services or referrals, and maintain communication with residents and families.
- Assist guests with financial and legal matters, such as applying for pensions, referrals to lawyers, and referrals to funeral homes for preplanning arrangements.
- Provide discharge planning services, including helping to place a guest on a waiting list for community congregate living, arranging intake for home care services, and assisting with transfer arrangements to other facilities.
- Provide or arrange needed counseling services and psychiatric medication management as indicated.
- Ensure documentation is accurate and timely in MDS care plans.
