Hospice Care Provider Position Overview

We are seeking an experienced Nurse Practitioner to join our Hospice team at PeaceHealth St. Joseph Medical Center in Bellingham, Washington.

This role offers the opportunity to provide compassionate care and support to patients and their families, addressing their physical, emotional, and spiritual needs.

As a member of our interdisciplinary team, you will work collaboratively with physicians, nurses, social workers, and other healthcare professionals to ensure seamless patient care.

We are looking for a candidate with experience working in hospice and palliative care, with a strong focus on symptom management and patient-centered care.

Key Responsibilities

  • Assess patients' physical, emotional, and spiritual needs, developing individualized care plans that address these needs.

  • Collaborate with other healthcare professionals to provide comprehensive care, including symptom management, pain control, and end-of-life care.

  • Maintain accurate and timely records of patient interactions, including progress notes, medication lists, and test results.

  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.

  • Participate in quality improvement initiatives to enhance patient outcomes and promote a culture of excellence within the organization.

Requirements and Qualifications

  • Bachelor's degree in Nursing (BSN) or Doctoral degree in Nursing (DNP/PhD)

  • Current RN licensure in the state of Washington

  • Board certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP-BC)

  • 3+ years of experience in hospice and palliative care, with a focus on patient-centered care and symptom management

  • Strong communication and interpersonal skills, with the ability to work effectively in a team environment
