Position Overview

As a dedicated Registered Nurse at TemPositions Health Care, you will deliver exceptional nursing services tailored to the individual needs of patients. Your role will involve managing and coordinating a comprehensive range of health care services for a designated group of patients.

Key Responsibilities

  • Conduct thorough assessments of patients' physical, social, and psychological needs, as well as their environmental conditions, utilizing your professional expertise to create effective care plans.
  • Engage in clear communication with the patient's physician, addressing any issues that may arise, such as abnormal clinical findings or medication discrepancies, to ensure optimal care.
  • Oversee and coordinate all aspects of your assigned patient caseload, ensuring a holistic approach to health care delivery.
  • Collaborate with multidisciplinary health team members, including physicians, occupational therapists, physical therapists, social workers, and family members, to establish, implement, and adjust care plans based on mutually agreed-upon goals.
  • Continuously assess and monitor the effectiveness of care plans, making necessary updates in collaboration with the health care team.
  • Supervise the care provided by Licensed Practical Nurses and other support staff, ensuring compliance with regulatory standards through regular home visits.
  • Document all communications and updates regarding patient care, including clinical notes and progress reports, to maintain a comprehensive record of patient health status.
  • Assist patients with daily living activities, which may involve physical support such as transferring and positioning patients of varying conditions.
  • Plan and facilitate safe discharge processes for patients, ensuring all necessary documentation and communication with the health care team and family members are completed in accordance with regulations.
  • Adhere to established workflows and procedures to maintain high standards of care.
  • Perform additional related duties as required to support patient care and organizational goals.

Qualifications

  • Current licensure and registration as a professional nurse in New York State is mandatory.
  • A minimum of one year of clinical nursing experience is required.
