Role Summary
The Registered Nurse in Home Health is responsible for evaluating and admitting patients into the Home Care program. This role involves developing individualized care plans and delivering care as per the established guidelines. The RN will act as a case manager, making necessary adjustments to the care plan based on changes in patient symptoms and needs.
The RN will participate in scheduled case conferences to review and plan patient care or discharge, ensuring all documentation is completed accurately. This position serves as a liaison between patients, caregivers, physicians, and community health partners, with a focus on optimizing care while managing costs related to mileage, supplies, and visit scheduling.
Additionally, the RN will complete all required documentation related to patient interactions, assessments, and care transitions, adhering to regulatory guidelines. The role includes providing on-call coverage for both Home Care and Hospice services as needed. The RN is expected to foster teamwork and promote exceptional patient and guest experiences while upholding safety and quality standards.
Qualifications
Education:
Associate's degree in Nursing and current licensure in the State of Alabama is required.
Experience:
A minimum of 2 years of experience in a hospital environment, specifically in Med/Surg, Critical Care, or Emergency Room settings. Previous experience in Home Care or Hospice is preferred.
Skills and Abilities:
The ideal candidate must demonstrate strong critical thinking and organizational skills. The ability to work independently and make informed decisions is essential. Flexibility in scheduling and the capacity to prioritize tasks to meet patient needs are also required.
