**Job Summary**
We are seeking a skilled Registered Nurse to join our team as a Patient Care Coordinator. In this role, you will be responsible for coordinating and delivering patient care utilizing the nursing process. You will work closely with the Director, Patient Logistics & Ancillary Services, and Nursing Service Admin to ensure the highest level of patient care.
**Responsibilities**
* Coordinate patient care activities, including medication administration, wound care, and vital sign monitoring
* Collaborate with healthcare team members to develop and implement individualized patient care plans
* Communicate effectively with patients, family members, and healthcare personnel to ensure seamless care transitions
* Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
**Requirements**
* Current American Heart Association (AHA) Healthcare Provider CPR card
* Ability to communicate effectively in English
* Strong organizational and time management skills
* Ability to work in a fast-paced environment with multiple priorities
