Job Summary:

The Advanced Practice Provider - Nurse Practitioner or Physician Assistant will be responsible for providing primary care services to our members, including assessing, diagnosing, and treating a variety of medical conditions. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each member.

Key Responsibilities:
  • Provide primary care services to members in long-term care settings
  • Assess, diagnose, and treat a variety of medical conditions
  • Develop and implement individualized care plans that meet the unique needs of each member
  • Collaborate with our interdisciplinary team to ensure high-quality, patient-centered care
  • Communicate effectively with members, families, and healthcare providers to ensure seamless care coordination
Requirements:
  • State unrestricted license as a Nurse Practitioner or Physician Assistant
  • National Board certification as an Adult, Family, Geriatric, or Primary Care nurse practitioner or Physician Assistant
  • DEA license
  • Active national provider number (NPI) and Medicare provider number
  • Valid state driver's license with a good driving record and proof of automobile insurance
Preferred Qualifications:
  • Experience working in long-term care settings
  • Geriatric or palliative care experience
  • Computer/technology proficiency
  • Excellent communication and interpersonal skills
