About the Role

This position offers a competitive salary of $65,000 per year, based on national averages for similar positions in the United States. The successful candidate will be responsible for delivering high-quality patient care directed by physicians as established in the home health plan of care, consistent with clinical best practices and resulting in high-quality, improved outcomes, and an exceptional patient experience.

Responsibilities
  • Provide one-on-one care to patients in their homes, utilizing clinical nursing knowledge, physical assessment, teaching, and procedural skills to deliver high-quality patient care.
  • Contribute to the development of the home health plan of care, working closely with multidisciplinary teams to ensure seamless care transitions.
  • Deliver patient care directed by physicians, adhering to established standards and guidelines while promoting patient safety and satisfaction.
Requirements

To succeed in this role, you'll need:

  • A graduate degree in Nursing from an accredited program and a valid RN license in the state(s) where you'll be providing care.
  • At least one year of experience caring for similar patient populations.
  • A valid driver's license and automobile insurance in accordance with state and organizational requirements.
  • Mandatory reporter training for child and dependent adult abuse.
  • Person-centered care (PCC) course completion within the first 12 months of hire and annual competency validation activities.
  • CPR certification with re-certification.
  • Basic knowledge of current medications, including modes of administration, actions, and side effects.
  • Competency in various nursing skills, including NPWT, complex wound care, IV therapy, catheter care, ostomy care, chronic disease management assessment, and teaching.
