Speech Therapist, Home Health: PT

As a Speech-Language Pathologist with Firelands Health, you will have the opportunity to work with a variety of patients in their homes, helping them to reach their maximum level of functioning. This role is ideal for someone who is passionate about providing excellent patient care and making a difference in the lives of others.
Responsibilities:
Treat individuals with communication disorders to assist in reaching their maximum level of functioning.
Provide direct service to home health patients, including speech, language, cognitive, and swallowing deficits, by means of recognized assessment and treatment procedures.
Instruct patients and/or family members in home treatment programs and assist them in adjusting to their communication disability.
Maintain adequate patient records and participate in patient care conferences as required.
Participate in conferences with the Home Health team, as needed, and maintain close working relationships with healthcare teams in both assessment and treatment activities.
Display positive patient/family/staff relations in the performance of duties and determine priority in workload per job description and pre-established performance goals.
Ensure that assignments are completed by required deadlines and demonstrate commitment to professional growth through pursuit of continuing education activities and/or professional development.
Requirements:
Master's degree in Speech-Language Pathology, Certificate of Clinical Competence, licensed or eligible for licensure in the State of Ohio.
One year of experience in a medical setting preferred.
Ability to understand and communicate effectively in both written and oral form, as well as possess a valid State of Ohio motor vehicle operator's license and be insurable under Firelands' auto policy.
Punctual attendance at assigned work location is required, and the ability to work in a safe and efficient manner, maintain an accident-free work place, and comply with OSHA regulations and CDC standard and transmission-based precaution recommendations and to utilize proper personal protective equipment.
