Position Overview:

The Clinical Social Services Coordinator plays a crucial role in managing the healthcare services provided to our members, collaborating with healthcare providers, families, and other resources as necessary.

This position will also assist in the deployment of case management software and contribute to the organization’s Utilization Management and Quality Management initiatives.


Key Responsibilities:
Ensure compliance with departmental competencies and standards

Patient-Centered Care:

Foster a supportive and healing environment that prioritizes the needs of patients and their families throughout their healthcare journey.

Work collaboratively with case management resources to develop and document treatment plans for members who qualify for case management services, coordinating with primary care providers and interdisciplinary teams as needed.

Conduct comprehensive assessments, including behavioral health, substance use, and psychosocial evaluations, to understand each member's health status, support systems, and prescribed treatments.

Engage with members to establish mutual goals and determine the frequency of follow-ups, providing education and resources tailored to their specific health conditions.

Empower members to advocate for their healthcare needs and assist them in formulating questions for their healthcare providers.

Maintain a detailed case management database for tracking referrals and treatment plans.

Collaborate with Medical Directors and other departments to address case-specific matters.

Utilize knowledge of local, state, and federal programs to connect members and their families with suitable community resources.


Qualifications:

Education:
Master's degree in Social Work required
Licensure:
LSW licensure in Ohio is mandatory; LISW preferred

Experience:
A minimum of 2 years in case management within behavioral health or related fields. Familiarity with medical terminology and basic computer skills is essential.
Strong communication skills and experience in medical social work are required.
Proven problem-solving, organizational, and time management abilities to thrive in a dynamic environment.

Demonstrated clinical, organizational, and analytical skills to work independently while adapting to changing regulatory landscapes.

Proficient in PC usage, including web navigation and research capabilities.

Excellent verbal, written, and interpersonal skills to facilitate collaboration across departments and maintain professionalism in challenging situations.

Ability to effectively implement and utilize case management software.
