Job Summary:
The American Home Health Agency is seeking a skilled and compassionate Registered Nurse to join our team as a Field Nurse. As a key member of our Home Health Team, you will be responsible for providing high-quality nursing care to patients in their homes, while also directing and supervising Home Health Aides.
Key Responsibilities:
- Assess and Plan Care: Conduct thorough assessments of patients' physical and emotional status, level of competency, home environment, and safety factors to develop individualized care plans.
- Provide Skilled Nursing Care: Administer medications, perform wound care, and provide other skilled nursing interventions as needed.
- Supervise Home Health Aides: Direct and supervise Home Health Aides in providing non-skilled care to patients, ensuring that care is delivered in a safe and effective manner.
- Communicate with Patients and Families: Educate patients and families on disease management, medication adherence, and other health-related topics.
- Collaborate with Healthcare Team: Work closely with physicians, therapists, and other healthcare professionals to ensure comprehensive care and coordination.
- Document Patient Information: Accurately document patient information, including assessments, care plans, and progress notes.
- Participate in Quality Improvement: Contribute to quality improvement initiatives, including surveys, audits, and other activities to ensure high-quality care.
Requirements:
- Education and Licensure: Graduate of an accredited School of Nursing and licensed in the state of Florida.
- Experience: At least 1 year of experience as a Registered Nurse in a home health or hospice setting.
- Skills: Strong assessment and planning skills, excellent communication and interpersonal skills, and ability to work independently and as part of a team.
Physical and Mental Demands:
- Physical Demands: Ability to transfer, lift, or assist patients with an average weight of 160 pounds, with or without the aid of mechanical devices.
- Mental Demands: Ability to concentrate on detail with frequent interruptions, follow daily routines and requirements, and cope with the mental and emotional stress of the position.
Working Conditions:
The Field Nurse will work primarily in patients' homes, with occasional visits to the agency's office. The ability to spend 80% of the workday standing and/or moving about is required.
