You will be responsible for managing the coordination of care for veterans traveling between facilities, tracking consults, and monitoring transfers. Key Responsibilities

  • Manage the coordination of care for veterans traveling between facilities
  • Track consults and maintain accurate data logs
  • Monitor transfers and ensure seamless transitions
  • Collaborate with local and remote providers, VISN leaders, and other Traveling Veteran Coordinators
