Compassionate Patient Care Advocate

We are seeking a compassionate Registered Nurse (RN) to join our Bristol Hospice Team. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to patients and their families in a home setting.
The ideal candidate will have at least one year of acute care experience in an institutional setting and be licensed to practice nursing in the state. A strong understanding of the hospice concept and the needs of the terminally ill is essential. You must be flexible in work hours and able to travel throughout the Bristol Hospice Service Area.
Key Responsibilities:
- Complete comprehensive assessments of patients and their families to determine hospice needs.
- Develop and implement individualized care plans that establish goals and incorporate palliative nursing actions.
- Administer medications and treatments as prescribed by physicians and counsel patients and families on meeting nursing and related needs.
- Provide health care instructions to patients and assist them with activities of daily living.
- Communicate effectively with patients, families, and healthcare professionals to ensure continuity of care.
