As a Hospice Admission Nurse with LifeBridge Health, you will play a critical role in ensuring that patients receive high-quality, compassionate care in their final stages of life. You will assess patients' and families' needs, develop and implement individualized care plans, and collaborate with the interdisciplinary team to provide seamless care.
Responsibilities- Conduct thorough assessments of patients' physical, emotional, and spiritual needs
- Develop and implement individualized care plans that address patients' unique needs and goals
- Collaborate with the interdisciplinary team to ensure continuity of care
- Provide education and support to patients and their families
- Document patient progress and outcomes
- Associate's degree in nursing
- Minimum of two years of recent medical/surgical nursing experience
- One year of home health experience preferred
- Expertise in performing health assessments
- CHPN certification within four years of hire
