Position Overview: The Hospitalist is accountable for delivering comprehensive internal medicine services to patients across various healthcare settings. This role involves creating a well-coordinated care plan that integrates information from medical records, patient and family input, and collaboration with primary care physicians and specialists.

Key Responsibilities:

  1. Hospital Rounds: Conduct daily rounds in the hospital from Monday to Friday, ensuring all patients are evaluated consistently.
  2. Collaboration: Work closely with the Hospital, Care Management Team, and fellow Hospitalists to guarantee that all patients receive daily attention.
  3. Patient Admissions: Admit patients from the Emergency Room during the afternoon as needed, particularly when assigned to a 'float' position.
  4. Care Transition: Collaborate with Case Managers to facilitate the transfer of patients from external hospitals into the network.

Clinic Responsibilities:

  1. Manage follow-up care for patients discharged from the hospital until they reach stabilization.
  2. Assess patients who have experienced falls.
  3. Provide pre-operative evaluations for patients undergoing surgeries requiring anesthesia.
  4. Support Nurse Practitioners by reviewing cases collaboratively.

Skilled Nursing Facility Duties:

  1. Visit skilled patients in Skilled Nursing Facilities (SNFs) weekly until they no longer require skilled care.
  2. Participate in weekly SNF meetings to discuss patient cases.

Comprehensive Care Planning: Develop a thorough and coordinated care plan that incorporates insights from medical records, patient and family feedback, and input from primary care physicians and specialists as necessary.

  1. Documentation: Ensure timely completion of medical records, including:
  • History & Physicals within 24 hours of admission.
  • Discharge plans coordinated with the Care Management Team on the discharge day.
  • Clinic visit notes documented on the same day as the visit.
Communication: Maintain open lines of communication with patients, their families, and the Primary Care Physician regarding care plans and any changes in health status.Care Transition Support: Collaborate with the Care Management Team to ensure seamless transitions of care across different settings.

On-Call Duties: Participate in an on-call schedule that varies by location.

Qualifications:

  1. Completion of an Internal Medicine Residency.
  2. Medical Doctorate degree.
  3. Valid Medical License in the applicable state.
  4. DEA certification.
  5. Board certification or eligibility in the specialty is required.
  6. A minimum of 2-3 years of experience as a Hospitalist is preferred.
