About the Role:

As a Physical Therapist with Adoration Home Health and Hospice, you will play a vital role in providing high-quality patient care in the comfort of their own homes. Our team is dedicated to delivering compassionate and personalized services that empower patients to live with dignity and fulfillment.

Key Responsibilities:

  • Provide physical therapy services to patients according to a written physician's plan of care
  • Initiate physical therapy programs and instruct other personnel and/or family/caregiver members as appropriate
  • Prepare and submit clinical and progress summaries
  • Provide in-service education programs for nursing organization personnel as needed
  • Participate in discharge planning
  • Supervise Physical Therapy Assistants according to policy and state regulations
  • Ensure compliance with all state and federal legal and regulatory requirements

Requirements:

  • Graduate of a physical therapist education program approved by CAPTE with current licensure to practice in state of operation
  • 2 years of experience in adult physical therapy
  • Home health experience preferred
  • Commitment to clinical and documentation excellence
