Job Summary:

The successful candidate will provide exceptional behavioral healthcare services to clients of all ages, collaborating with primary health care providers, specialists, psychiatrists, dependency programs, and other healthcare professionals.

Responsibilities:
  • Assess patients and develop a list of actual and potential needs/diagnoses.
  • Plan and implement patient care, ensuring accuracy in communications and timely, correct care is provided.
  • Provide outstanding behavioral healthcare services, including gathering health history, maintaining patient documentation, and ensuring patient calls are returned and follow-ups are handled promptly.
  • Collaborate with healthcare professionals to provide comprehensive care.
Requirements:
  • Twelve (12) continuous months of experience within the last three (3) years rendering direct patient care in public or private institution.
  • Valid Photo ID, Driver's License or Passport.
  • Possess current Basic Life Support (BLS) certification from an American Heart Association.
  • Valid California Social Worker license.
