St. Luke's Hospital is seeking a skilled Home Health Registered Nurse to join our team. As a key member of our interdisciplinary team, you will work collaboratively to deliver high-quality nursing services to patients in their homes and places of residence.
Responsibilities- Evaluate and perform comprehensive assessments on patients referred for home health nursing services, determining eligibility for admission and formulating initial care plans.
- Refer patients to other agency disciplines and community resources as needed, promoting health and well-being through teaching, counseling, and preventive measures.
- Provide ongoing assessments to determine service frequency, change in care plans, and discharge from service, ensuring compliance with agency policy and philosophy.
- Function as the designated coordinator of patient care, responsible for assuring the provision of optimum, comprehensive multidisciplinary care.
- Communicate with patients' physicians, referral sources, and multidisciplinary team members both orally and in writing, as required by patients' conditions and agency policy.
- Maintain and use patient clinical records, reports, and written communications according to agency policy and directives.
- A graduate of an approved school of professional nursing, licensed as a Registered Nurse by the state in which practicing, with a Baccalaureate degree preferred.
- Two years of experience as a professional nurse in nursing practice or previous community health experience as a professional nurse preferred.
Heavy work exertion, including lifting and carrying 100 lbs, standing and walking for at least 6 hours in an 8-hour workday, and exposure to contagious diseases, road hazards, and domestic pets.
