About the Role

VNS Health is seeking a skilled Physical Therapist to join our team as a Float Physical Therapist. As a key member of our interdisciplinary care team, you will provide personalized rehabilitation therapy to patients in their homes or care facilities.

Key Responsibilities
  • Conduct comprehensive assessments of patients to determine their needs and develop effective treatment plans.
  • Deliver high-quality rehabilitation therapy to patients, constantly evaluating their evolving needs and adjusting treatment plans accordingly.
  • Practice independently in the community, working collaboratively with other healthcare professionals to ensure seamless care.
Requirements
  • Minimum of one year of physical therapy experience or successful completion of a probationary period as a VNS Health Rehab Intern.
  • Licensure to practice as a Physical Therapist in New York State.
  • Valid driver's license required for all areas outside of Manhattan.
