Key Responsibilities:
  • Assist in the admission and discharge process, ensuring a smooth transition for residents and their families.
  • Complete accident and incident reports, and participate in the investigation process to identify areas for improvement.
  • Transcribe physician orders and document resident information accurately, notifying physicians and family members as needed.
  • Review medication orders for completeness and accuracy, and prepare and administer medications as ordered by physicians.
  • Use electronic medical records effectively to maintain accurate and up-to-date resident information.
  • Participate in care planning and conferences, updating care plans as needed to ensure resident needs are met.
  • Develop and maintain positive working relationships with all disciplines to ensure resident needs are met.
  • Participate in continuing education programs to stay current with best practices and industry developments.
Requirements:
  • Current LPN license active in the state of Wisconsin.
