About the Role

Amergis is seeking a highly skilled and compassionate Long Term Care Registered Nurse to join our team. As a Long Term Care Registered Nurse, you will be responsible for providing high-quality patient care to individuals in a Long Term Care and/or Assisted Living Facility.

Key Responsibilities
  • Assess and Plan Patient Care: Utilize the nursing process to assess, plan, implement, and evaluate patient care in a Long Term Care/Assisted Living Facility.
  • Conduct Focused Interviews: Perform focused interviews to identify specific needs and assess signs and symptoms indicating physiologic and psychological changes in the patient's condition.
  • Collect and Analyze Data: Collect, analyze, and interpret data and information from healthcare team members and document actual and/or potential nursing diagnoses.
  • Develop and Implement Care Plans: Document the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions as well as responses to interventions.
  • Provide Safe and Comfortable Environment: Perform interventions according to identified priorities, plan of care, and the facility policies and patient outcome standards, and provide a safe, comfortable, and therapeutic patient environment.
  • Initiate and Evaluate Patient Education: Initiate and evaluate patient and/or family education, and use clinical judgment in evaluation activities to meet patient care needs of an assigned floor/unit.
  • Monitor Safety and Effectiveness: Monitor safety and effectiveness of the environment and equipment, and participate in activities to facilitate good interpersonal communications with the chosen facility.
  • Support Professional Practice: Support professional practice by observing the following: uses knowledge and practices in accordance with 1) the Nurse Practice Act; 2) Standards of Nursing Practice; 3) Standards of Patient Care; 4) Joint Commission, state and local laws; 5) Patient's Bill of Rights; and 6) Amergis Healthcare, nursing and client hospital policies.
Requirements
  • Current Registered Nurse License: Current Registered Nurse License for the state in which the nurse practices.
  • Compliance with Professional Standards: Complies with all relevant professional standards of practice.
  • Participation in Competency Program: Participation and completion of Amergis' Competency program when applicable.
  • CPR Certification: Current CPR if applicable.
  • Health Certificate: Current Health certificate (per contract or state regulation).
  • Meet Federal, State, and Local Requirements: Must meet all federal, state, and local requirements.
  • Successful Completion of New Hire Training: Successful completion of new hire training as applicable to job site.
  • Understand Patient Confidentiality: Understand patient confidentiality and HIPAA requirements.
  • Effective Communication Skills: Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required.
  • Computer Proficiency: Computer proficiency required.
  • Age Requirement: Must be at least 18 years of age.
