We are seeking a skilled and compassionate Home Health Physical Therapist to join our team. As a Home Health Physical Therapist, you will be responsible for providing medically prescribed physical therapy to patients in the comfort of their own homes.

Estimated Salary: $85,000 - $110,000 per year

Job Description:

As a Home Health Physical Therapist, you will work closely with patients, families, and healthcare teams to develop and implement personalized plans of care. Your goal is to help patients achieve optimal health and well-being by addressing physical limitations and promoting independence.

Responsibilities:

  • Evaluate patients' physical abilities and develop treatment plans to address specific needs.
  • Conduct physical therapy sessions in patients' homes, teaching exercises, techniques, and strategies to promote recovery and mobility.
  • Collaborate with healthcare teams to coordinate care and ensure seamless transitions between settings.
  • Maintain accurate records of patient progress, goals, and treatment plans.
  • Communicate effectively with patients, families, and healthcare professionals to ensure informed decision-making.
  • Participate in quality improvement initiatives and contribute to the development of best practices in home health physical therapy.

Requirements:

  • Current Florida Physical Therapist License.
  • CPR certification.
  • Valid driver's license and reliable transportation.

Working Conditions:

This role requires working in a variety of home environments, potentially exposing you to biological hazards. However, we prioritize your safety and provide necessary precautions.
