Position Overview

Licensed Practical Nurse – Adult Services

Schedule: Monday to Friday, day shift with flexibility

Compensation: $27.75 per hour

We are currently seeking a full-time Licensed Practical Nurse to support the healthcare needs of individuals within our Residential Services Program.

Key Responsibilities

The Residential LPN will:

  • Monitor the health requirements of participants, ensuring all medical needs are addressed.
  • Maintain accurate medical records, including MARs, Bowel Charts, Blood Sugar Flow Sheets, and Weight Records.
  • Document nursing notes as necessary.
  • Assist with medical filing as directed by Registered Nurses.
  • Oversee the proper storage and management of medications, including disposal of empty blister packs and checking expiration dates on OTC medications.
  • Complete medication orders and verify formularies for all medications.
  • Ensure that doctor's orders are in place for each medication, treatment, and adaptive equipment.
  • Accompany individuals to medical appointments as required, completing necessary documentation.
  • Administer medications to individuals who are unable to self-medicate.
  • Order medical supplies as needed.
  • Conduct medication regimen reviews and self-medication assessments under RN supervision.
  • Assist the Nursing Coordinator in developing and implementing healthcare policies.
  • Provide health maintenance counseling to participants and staff, offering training as appropriate.
  • Act as a liaison to community resources, including physicians, laboratories, and day programs.
  • Perform additional nursing tasks as permitted under OPWDD's Scope of Practice for LPNs, following training and competency validation.
  • Adhere to the agency's Code of Legal and Ethical Behavior, ensuring responsibilities are performed with integrity and respect.
  • Follow all agency policies and procedures, maintaining a focus on personal and others' safety.
  • Complete other job-related activities as requested by supervisors.
Physical Requirements

The physical demands of this role include:

  • Ability to lift up to 50 lbs, bend, squat, and grasp with both hands.
  • Using appropriate techniques to lift individuals, including the use of assistive devices.
  • Providing physical assistance for ambulation, utilizing equipment as necessary.
  • Standing and walking for extended periods.
  • Pushing, pulling, and maneuvering adults in wheelchairs.
  • Effective verbal and written communication skills in English for regulatory documentation.

Qualifications
  • Must hold a valid LPN license from the NYS Department of Education.
  • Experience working with individuals with developmental disabilities is preferred.
  • Proficient computer skills are required.
  • Ability to stand and walk for long durations.
  • Capability to assist individuals in wheelchairs and perform physical tasks as necessary.
  • Valid NYS driver's license that meets insurance requirements.
