About the Role

We are seeking a skilled Nurse Practitioner to join our team in providing high-quality care to our patients in the comfort of their own homes. As a PRN/Part Time Nurse Practitioner, you will conduct comprehensive health assessments, develop care plans, and provide education to patients and their families.

Responsibilities
  • Conduct home visits to assess patients' health and develop personalized care plans
  • Collaborate with healthcare teams to ensure seamless care coordination
  • Provide education and support to patients and their families to promote healthy behaviors and disease management
  • Document patient information and care plans accurately and efficiently
Requirements
  • Master's degree in Nursing or related field
  • Current RN and NP licensure in the state of practice
  • Board certification in a relevant specialty
  • Minimum 1 year experience as a Nurse Practitioner
  • Strong communication and interpersonal skills
