We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our Primary Care PACT Teams at the Veterans Affairs, Veterans Health Administration. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.
Key Responsibilities- Provide complex age-specific nursing care for adult and geriatric inpatients or outpatients with physical, mental, behavioral, or psychosocial needs.
- Work collaboratively with the nursing and treatment teams to plan and implement a full range of practical nursing care for veterans.
- Assist with data collection for outpatients and new admissions, observing, reporting, and documenting signs and symptoms of patient's general condition.
- Participate in nursing and/or interdisciplinary team conferences, exchanging information about patient needs and condition, and assist in the nursing management of patients with complex problems.
- Provide input into the development, implementation, and evaluation of the Nursing Care Plan and/or the Interdisciplinary Plan of Care.
- Graduation from a school of practical nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC).
- Full, active, current, and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
- Ability to observe, identify, and respond to the patient's needs for nursing, medication, and equipment-assisted care, and to recognize deviations from normal.
- Knowledge and ability to recognize the need for and to institute emergency measures when indicated, promptly seek the assistance of the RN or MD/DO, and assist in resuscitation procedures in cardiac and/or pulmonary arrest.
- Experience working in an outpatient clinic setting.
- Ability to work effectively with patients, families, and other staff members.
- Knowledge of human behavior, motivations, and reactions to situations.
