Key Responsibilities- Provide high-quality patient care to residents in a sub-acute and long-term care setting
- Collaborate with Center Leadership to achieve AlignMed Partners' mission of delivering high-quality healthcare and everyday compassion
- Work collaboratively with Medical Directors and Attending Physicians to ensure seamless patient care
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments
- Graduate from an accredited School of Nursing program and licensed as a registered nurse
- Certified as a Nurse Practitioner by being a nursing graduate from an accredited Nurse Practitioner Program with a Master's Degree in Nursing and a concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP)
- Minimum of two years of experience working with adult or geriatric populations
- APRN with an unrestricted professional license and DEA, for each state in which they practice
- Comprehensive 90-day training program and continued support
- New and recent graduate mentorship and clinical education program
