Job Summary

We are seeking a skilled Ambulatory Clinical Nurse Specialist to join our team at AdventHealth Rocky Mountain. As an integral member of our healthcare team, you will play a vital role in delivering exceptional patient care and contributing to the success of our organization.

About the Role

The Ambulatory Clinical Nurse Specialist will be responsible for assessing patient needs, developing individualized care plans, and implementing evidence-based interventions to optimize patient outcomes. You will work closely with the interdisciplinary team to ensure seamless care coordination and communication. Your expertise in community health concepts, ambulatory clinical nursing care, case management, and care coordination will be invaluable in this role.

Responsibilities

Your primary responsibilities will include:
  • Assessing patient physical, psychosocial, cultural, and spiritual needs through observation, interview, and review of medical records.
  • Developing and implementing patient care plans in collaboration with the interdisciplinary team.
  • Evaluating patient response and intervening to ensure optimal patient outcomes.
  • Demonstrating competency in therapeutic/diagnostic interventions, including IVs, incision/wound care, medication administration, catheterization, and specimen collections.

Requirements

To be successful in this role, you will need to have the following qualifications:
  • A graduate degree in nursing from an accredited institution.
  • A valid Colorado RN license or Compact RN license from a NCLA Compact State.
  • BLS certification from the American Heart Association.
