Nurse Practitioner

SNCH is seeking a highly skilled Registered Nurse to join our team as an RN Resident. The successful candidate will be responsible for providing exceptional patient care, working collaboratively with our interdisciplinary team, and contributing to a safe and efficient healthcare environment.
Responsibilities:
• Provide direct patient care, including administering medications, performing various treatments, and documenting patient information.
• Participate in patient education, promoting health literacy and empowering patients to take an active role in their care.
• Collaborate with colleagues to develop and implement individualized care plans, ensuring seamless transitions and optimal patient outcomes.
• Stay up-to-date with current evidence-based practices, policies, and procedures, applying this knowledge to improve patient care and outcomes.
Requirements:
• Current licensure as a Registered Nurse in New York State.
• Baccalaureate degree in Nursing or higher.
• Completion of an accredited nursing program.
• Strong interpersonal and communication skills, with the ability to work effectively in a fast-paced environment.
