Job Summary

We are seeking a skilled Care Management Specialist to provide high-quality care management to our members. As a Behavioral Health Care Manager, you will be responsible for outreach and engagement, case management, coordination of care, resource linkages, and ensuring seamless transitions of care.

Main Responsibilities:
  • Provide effective care management for assigned members, including care coordination, linkage to resources, support in transitions of care, facilitating member self-efficacy and self-management.
  • Engage and collaborate with staff outside the BH department to provide integrated care to assigned members.
  • Conduct comprehensive, holistic assessments of members' health and wellness needs.
Requirements:
  • Bachelor's degree in social work or related field preferred; Master's degree in social work or counseling is a plus.
  • Minimum 1-2 years of experience in a social service setting.
  • Valid, unrestricted state licensure (LCSW, ACSW, LMFT, or AMFT).
  • Current and valid Driver's License.
  • Proof of auto insurance.
Compensation and Benefits:
  • $65,000 - $80,000 per year, depending on experience.
  • Paid Time Off: 12 holidays and up to 3 weeks of accrued PTO.
