Key Responsibilities:
1. Collaborate in the formulation and upkeep of a personalized, goal-oriented care plan, engaging with other healthcare professionals to ensure the needs of patients and caregivers are effectively addressed.
2. Monitor, evaluate, document, and communicate patient responses to treatments and educational initiatives, adjusting care priorities based on patient and caregiver requirements. Collect and relay pertinent data regarding the patient's physical, psychological, environmental, self-care, educational, and discharge planning needs to the responsible physician or registered nurse.
3. Actively participate in departmental and unit enrichment activities that promote positive outcomes, including committee assignments, continuous quality improvement initiatives, and the orientation and training of new staff.
4. Administer a comprehensive range of treatments and therapies, including respiratory care, suctioning, medication administration, cardiac monitoring, and intravenous infusion management, while providing patient and caregiver education based on established teaching protocols.
5. Assist patients and families during their admission process, orienting them to the clinical environment and facilitating a smooth discharge.
6. Offer emotional support to patients and families, employing verbal de-escalation techniques for anxious or potentially aggressive individuals and promoting age-appropriate coping strategies.
Educational Qualifications:
* Completion of a state-accredited practical nursing program is required.
Licensure/Certification:
* Must hold a valid Practical Nurse (LPN) license in North Carolina and possess Basic Life Support (BLS) certification.
Professional Experience:
* A minimum of two years of experience as a Licensed Practical Nurse is required.
