We are seeking a skilled Hospice RN to join our team at Jobot. As a Hospice RN, you will play a vital role in providing high-quality care to patients in need.
Key Responsibilities:
- Coordinate care for patients in hospice, ensuring their needs are met comprehensively.
- Assess patients' needs, symptoms, and overall condition regularly to develop and adjust care plans accordingly.
- Offer support and guidance to patients and their families, addressing concerns and providing education about the hospice process.
- Work closely with physicians, other healthcare professionals, social workers, and counselors to ensure a holistic approach to patient care.
Requirements:
- Experience in hospice care is highly preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
