Job Description

At Desire Home Care, Inc., we are seeking a skilled and compassionate Nurse Care Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and ensuring the best possible outcomes for our clients.

Key Responsibilities:

  • Patient Assessment: Conduct thorough assessments of patients' physical and emotional needs to develop personalized care plans.
  • Care Planning: Collaborate with patients, families, and healthcare providers to create and implement effective care plans that address patients' unique needs and goals.
  • Medication Management: Administer medications and treatments as prescribed by physicians and monitor patients' responses to ensure optimal care.
  • Communication: Maintain open and effective communication with patients, families, and healthcare providers to ensure seamless care coordination and address any concerns or issues that may arise.
  • Documentation: Accurately and thoroughly document patient information, including assessments, care plans, and progress notes, to ensure continuity of care and compliance with regulatory requirements.
  • Team Collaboration: Work collaboratively with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive and coordinated care.

Requirements:

  • Education: Graduate of an accredited nursing program.
  • License: Current RN licensure in the state of practice.
  • Experience: Minimum of two years of experience in home health or community health nursing, with a focus on patient care coordination and management.
  • Skills: Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers. Strong assessment and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.

Benefits:

  • Competitive Salary: $105-$110 per visit for RN Start of Care, $80-$100 per visit for Recert/Resumption of Care, $80 per visit for Discharge, $60-$70 per visit for RN Supervisory Visit/Follow Up Visit, and $40-$45 per visit for Non-Visit DC Oasis.
