Job Summary

We are seeking a highly skilled and experienced Clinical Team Lead to join our team at Southeastern Integrated Care LLC. The successful candidate will be responsible for supervising and managing team operations, ensuring that services are provided to meet the clinical needs of our members served.

Key Responsibilities
  • Provide individual therapy and behavioral interventions to recipients served by the team.
  • Designate team staff to apply specialized clinical expertise as clinically indicated for each recipient.
  • Develop and implement individualized supervision plans for team members.
  • Provide clinical supervision and guidance to team members.
  • Monitor and evaluate services, interventions, and activities provided by the team.
  • Facilitate weekly team meetings and ensure effective communication among team members.
  • Develop and implement relapse prevention and disease management strategies to support recovery.
  • Provide psycho-education to recipients, families, caregivers, and other individuals involved with the recipient.
  • Perform Intensive Case Management functions, including linking and arranging for services and referrals.
  • Participate in the initial Person-Centered Plan (PCP) and revision as needed.
  • Ensure and monitor the implementation of the PCP.
  • Participate in a first responder on-call system and coordinate first response resources according to consumer needs.
  • Work closely with other clinical/professional staff to maintain communication and provide feedback.
  • Conduct and supervise formal investigations into incidents/allegations of abuse, neglect, exploitation, or other circumstances that may present a risk to the safety and health of the person supported.
  • Ensure timely and accurate information when requesting authorizations from the LME/MCO/Statewide Vendor.
  • Notify appropriate parties upon the denial/modification of continued services and provide person-supported/guardian DMA-approved appeal policies and materials.
  • Monitor utilization of service to ensure effectiveness, appropriateness, and adherence to limits set forth in rules, PCP, and service authorization.
  • Coordinate transition to another level/type of care for the person supported.
  • Coordinate and oversee the discharge planning process, including the development of a discharge plan and follow-up resources.
  • Facilitate relationships and serve as a link between the company, consumer, guardians, local agencies, and the community.
  • Draft responses to and implement changes required by Medicaid, the Department of Facility Services, and/or other regulatory agencies.
  • Perform all other duties as reasonably required and assigned.
Requirements
  • Masters degree in human services-related field required.
  • Minimum of one (1) year of documented prior experience working with the population served with MH/SA diagnosis.
  • Ability to maintain strict confidentiality and possess effective communication/documentation skills.
  • Ability to learn and use personal computers and understand word processing and certain spreadsheet programs.
  • Successfully complete other training as may be required or amended by company policy.
  • Develop organizational and communication skills that foster teamwork.
  • Requires flexibility in scheduling and availability to meet service needs of the consumer/family and other team members.
  • Must have reliable transportation and be willing to travel locally.
  • Must meet 10A NCAC 27G.0104.
