About Community Health Network
  • Community Health Network was established by local residents, focusing on serving our community. Over six decades later, the essence of 'community' remains central to our mission. This involves delivering the highest quality care to our neighbors, supported by advanced technology. It also means engaging with the communities we serve through various volunteer initiatives and benefit programs.

We prioritize the growth and development of our dedicated caregivers, ensuring they remain at the forefront of their profession to better serve our patients.

Ultimately, it is about providing outstanding care in a straightforward manner — and we rely on your expertise to achieve this.

Impactful Role
  • Community North is seeking a part-time LPN for the Behavioral Health Senior Adult Unit. The Licensed Practical Nurse (LPN) will deliver both direct and indirect patient care under the guidance of a Registered Nurse, collaborating on patient care plans. Responsibilities include assisting with activities of daily living (ADLs), administering medications, and performing phlebotomy. The LPN will also engage in indirect care tasks such as order transcription, documentation, and reinforcing education for patients and their families. Collaboration with other team members to fulfill additional unit services as directed is essential.

Qualifications and Skills
  • Must be a graduate of an accredited practical nursing program, or a non-accredited program with a minimum of three years of relevant nursing experience.
  • Must hold a valid Licensed Practical Nurse (LPN) license issued by the Indiana Professional Licensing Agency.

At Community Health Network, we cultivate teams that provide exceptional care through empathy, effective communication, and teamwork. We view every member as a vital contributor to the outstanding patient experience. We take pride in referring to our staff as Caregivers rather than employees. Join us in making a positive impact in your community.
