Job Summary

Key Responsibilities
  • Provide direct patient care, including triaging, verifying medical information, and administering medications as prescribed by physicians.
  • Collaborate with healthcare teams to develop and implement patient care plans, ensuring seamless communication and coordination of care.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and implementing interventions to enhance patient outcomes.
  • Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices that promote patient safety and satisfaction.
Requirements
  • Current LPN license in good standing.
  • Nursing diploma or degree from an accredited institution.
  • Basic Life Support (BLS) certification, obtained within 90 days of hire.
Preferred Qualifications
  • At least 18 months of prior LPN work experience.
