This role is a PRN, on-call position requiring Saturday availability once every four weeks.

The Nurse LPN/RN in our Outpatient Clinic is an integral member of our person-centered integrated care team, delivering specialized office-based services to primary health care patients. This position plays a crucial role in addressing the comprehensive physical and behavioral health care needs of our patients through a holistic approach.

KEY RESPONSIBILITIES


• Prepare patients for examinations and treatments, including taking histories and vital signs, and documenting in patient records.

• Set up examination and treatment rooms with the necessary instruments and supplies.

• Conduct venipunctures, urinalysis, EKGs, and other procedures as required.

• Administer immunizations and injections as needed.

• Process patient prescriptions and maintain accurate documentation.

• Educate and instruct patients on health management.

• Ensure proper maintenance and sterilization of supplies and equipment.

• Assist healthcare providers in minor surgical procedures and physical examinations.

• Help schedule tests and treatments for patients.

• Screen calls for appropriate referrals and relay messages as necessary.

• Maintain patient files, records, and other pertinent information.

• Adhere to standard safety precautions, including the use of protective barriers and proper waste disposal.

• Comply with CLIA and OSHA regulations.

• Contribute to the cleanliness of the unit and its equipment.

• Monitor and manage medical supplies and laboratory needs, placing orders when necessary.

• Collaborate with medical providers regarding significant health information.

• Provide education to patients on medical conditions, treatments, medications, and self-management strategies.

• Monitor health information technology tools for alerts related to treatment and medication, as well as hospital admissions and discharges, to facilitate pre-visit planning.

• Triage medical inquiries and medication refill requests in accordance with agency policies and provider preferences.

• Serve as a nursing consultant to clinical staff and patients.

• Follow up with patients discharged from the hospital within three days to review discharge instructions, reconcile medications, and assess the need for follow-up appointments with primary care providers.

• Seek culturally competent services to meet the diverse needs of our consumers and their families.
