Physician Job Description

At Theoria Medical, we're seeking a skilled Physician to join our team as a Medical Director. As a key member of our post-acute care team, you'll be responsible for providing high-quality, patient-centered care to residents in skilled nursing facilities.

Key Responsibilities:
  • Serve as Medical Director for skilled nursing facilities, participating in administrative and quality initiatives
  • Collaborate with Nurse Practitioners and Physician Assistants to manage day-to-day resident care
  • Provide comprehensive primary care services, including diagnosing and treating acute and chronic illnesses
  • Document patient care in Theoria's proprietary electronic health record system, ChartEasy
  • Participate in Theoria's Chronic Care Management, Transitional Care, RPM, and other medical quality initiatives
Requirements:
  • Must be able to supervise collaborating nurse practitioners and physicians assistants
  • Must be able to utilize electronic/online medical tools, including EHR systems and state/county medical systems
