About the Role

We are seeking a skilled and compassionate Patient Care Manager - RN to join our team at SunCrest Home Health - Covington. As a Registered Nurse, you will play a vital role in providing high-quality patient care and leading our clinical services team.

Key Responsibilities
  • Supervise and Coordinate Clinical Services: Oversee the delivery of patient care, ensuring that services are provided in accordance with physician orders and applicable laws and regulations.
  • Provide Clinical Services: Offer nursing care to patients in their homes, as prescribed by physicians and in compliance with agency policies.
  • Coordinate Patient Care: Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start of care visits.
  • Ensure Patient Needs are Met: Continuously assess patient needs and provide individualized care that is reasonable, meets home health eligibility criteria, and aligns with physician orders.
  • Review and Revise Care Plans: Daily review assessments and plans of care, consulting with clinicians as needed to ensure that care is appropriate and effective.
Requirements
  • Current RN Licensure: Hold a current Registered Nurse license in the state of practice.
  • CPR Certification: Possess current CPR certification.
  • Driver's License and Vehicle: Have a current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
