Job Summary:

The Care Partner will work under the direction of the Registered Nurse to provide basic patient care functions, including ADLs, ambulation, and vital signs. You will also assist with daily living activities and comfort measures, and perform additional advanced skills as required.

Responsibilities:
  • Provide patient care under the direction of the RN, including hygiene, nutrition, elimination, and physical comfort measures.
  • Assist with daily living activities, such as bathing, dressing, and grooming.
  • Perform vital signs, including temperature, pulse, and blood pressure.
  • Assist with ambulation and transfer of patients.
  • Participate in hourly patient rounding and respond to patient concerns.
  • Communicate effectively with patients, families, and healthcare team members.
  • Document patient care in the Electronic Medical Record (EMR).
Requirements:
  • High school diploma or equivalent required.
  • Current certification as a Certified Nursing Assistant (CNA) or Patient Care Technician (PCT) preferred.
  • Basic Life Support (BLS) certification required.
  • Ability to work individually and as part of a team.
  • Strong interpersonal and communication skills.
