Your Role

As a Neuro Rehab Licensed Master Social Worker, you'll facilitate the social work care plan for treatment episodes, proactively monitor clinical quality, and facilitate timely discharge planning. You'll also serve as a social worker to consumers and families, coordinating with psychiatry, physiatry, psychology, nursing, physical therapy, occupational therapy, speech language pathology, and therapeutic recreation teams.

Key Responsibilities

  • Facilitate real-time communication with treatment teams, external Nurse Case Managers, insurance adjustors, and external physicians
  • Advocate and liaison for consumers' rights and benefits
  • Collaborate with multidisciplinary teams to develop and implement comprehensive treatment plans

Work Schedule

32 hours per week, Monday-Friday, 8am-4:30pm (flexible schedule)

Requirements

  • Master's degree in Social Work
  • Three years of case management experience or related social work experience and full license (LMSW)
