Job Summary:
We are seeking a highly skilled Speech Language Pathologist to join our home health team. As a Home Health Speech Language Pathologist, you will play a critical role in developing and implementing patient plans of care, conducting assessments, and educating patients and families on therapy goals and self-care.

Key Responsibilities:

  • ">
  • Develop and implement patient plans of care in collaboration with the multidisciplinary team
  • Conduct patient assessments, document progress, and ensure patient goals are met
  • Educate patients and families on plan of care, therapy goals, safety practices, and self-care
  • Collaborate with the home health team to address patient needs and concerns
  • Stay up-to-date with industry developments and best practices in speech language pathology

Requirements:
To be successful in this role, you will need a graduate degree from an accredited Speech Language Program and an active SLP license in Michigan. Additionally, you will require at least one year of experience as an SLP within the last five years, good clinical judgment, strong interpersonal skills, resourcefulness, and compassion.
