We are seeking a skilled Licensed Practical Nurse to join our Home Health team at St. Luke's Hospital. As a member of our multidisciplinary team, you will provide high-quality nursing care to patients in their homes, promoting health, preventing illness, and rehabilitating patients to their optimal level of functioning.
Key Responsibilities- Provide patient-centered care, assessing patients' needs and developing individualized care plans to meet their unique needs.
- Conduct ongoing patient observations to determine service frequency, change in Plan of Care, and discharge from service in compliance with agency policy and philosophy.
- Communicate effectively with patients, families, caregivers, and healthcare team members to ensure seamless care coordination.
- Participate in the development and operation of the agency, contributing to the evaluation of service programs and identifying opportunities for improvement.
- Maintain accurate and detailed patient clinical records, reports, and other written communications according to agency policy and directives.
- Graduation from a School of Practical Nursing with state accreditation at time of graduation.
- A minimum of two years experience as an LPN.
