About the Role:
Optum is creating something new in healthcare by uniting industry-leading solutions to build an integrated care model that addresses physical, mental, and social needs. As a key member of our HouseCalls team, you will provide annual clinical assessments to patients in their homes, helping identify and reduce health risks.

What You'll Do:
Conduct comprehensive home visits, evaluating past medical history, reviewing systems, reconciling medications, taking vital signs, and performing physical exams. Identify diagnoses for care management and active medical management. Communicate findings with primary care physicians to inform potential gaps in care. Intervene in urgent and emergent situations, educate patients on disease processes, medication, and compliance.

Key Responsibilities:
- Conduct in-home assessments, including evaluations of past medical history and review of systems.
- Perform evidence-based practice screenings, including point-of-care testing as needed.
- Identify diagnoses for care management and active medical management in treatment furtherance.
- Formulate lists of current and past medical conditions using clinical knowledge and judgment and assessment findings.
- Communicate assessment findings to inform members' PCPs of potential gaps in care.
- Identify urgent and emergent situations and intervene appropriately.
- Educate members on topics such as disease process, medication, and compliance.

Requirements:
- Active NP or PA license or ability to obtain by start date, unencumbered and free of any open disciplinary actions.
- For NPs: Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification.
- For PAs: Physician Assistant national certification through NCCPA.
- Ability to complete physical requirements of the job, including lifting 30-pound bags and navigating various dwelling conditions.
- Reliable transportation for home visit assessments.

Preferred Qualifications:
- 1+ years of clinical experience in highest level of education, clinical setting.
- May be requested to obtain additional licensure in other geographic areas.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home Health care or home visit experience.
- Ability to travel throughout the state, up to 5% of time, based on business needs.

Benefits:
