We're seeking a compassionate and dedicated Home Health Aide to provide exceptional care to our clients in the comfort of their own homes. As a Home Health Aide with CareConnect, you will play a vital role in delivering high-quality care that meets the unique needs of each patient.

Key Responsibilities:

  • Provide personalized care to patients, including bathing, dressing, and grooming, to ensure their safety and well-being.
  • Assist with mobility and transfers, using proper techniques to prevent injuries and promote independence.
  • Administer medications as directed by healthcare professionals, maintaining accurate records and reporting any concerns.
  • Monitor vital signs and report any changes or abnormalities to the appropriate medical personnel.
  • Offer emotional support and companionship to patients and their families, fostering a sense of comfort and trust.
  • Maintain a clean and safe environment for patients, adhering to infection control protocols and ensuring a healthy living space.

Requirements:

  • Certified Personal Care Assistant (PCA) or Certified Home Health Aide (HHA) certification.
  • Understanding of HIPAA regulations and the importance of patient confidentiality.
  • Ability to cook simple meals and perform light housekeeping duties as needed, promoting independence and dignity.
  • Satisfactory completion of New York State Health Department-approved HHA Training Program or equivalency.
  • Demonstrated adequate literacy and simple arithmetic skills, ensuring effective communication and record-keeping.
  • Must be at least 18 years old, with a strong commitment to delivering exceptional care.
