MEDSTAR HEALTH is seeking a highly skilled and compassionate Social Worker to join our team. As a Social Worker, you will play a vital role in providing social work services to our patients and clients, ensuring they receive the support and care they need to achieve optimal health outcomes.

Key Responsibilities:

  • Provide counseling and linkage with community services to patients and clients as needed.
  • Perform assessments to identify social and emotional needs, and develop plans to address these needs.
  • Collaborate with healthcare teams to understand the psychosocial and emotional factors related to patient health problems.
  • Document all aspects of care provision in accordance with state and federal regulations and agency policy.
  • Communicate effectively with other care providers to ensure seamless care coordination.
  • Participate in multidisciplinary quality and service improvement teams to drive excellence in patient care.
  • Represent the department and agency in community outreach efforts and link with needed community resources.

Requirements:

  • Master's degree in social work from a school accredited by the Council on Social Work.
  • 1-2 years of social work experience in a healthcare facility or setting.
  • Licensed Graduate Social Worker (LGSW), Licensed Certified Social Worker (LCSW), LCSW-C, LISW, or LICSW.

Preferred Qualifications:

  • Home Care experience.
