Key Responsibilities:
- Lead the Care Team: Supervise and coordinate the care provided by Licensed Practical Nurses (LPNs), State Tested Nursing Assistants (STNAs), and other team members to ensure the highest level of care for our residents.
- Medication Management: Provide safe and accurate medication-related interventions to our residents, ensuring their health and safety.
- Assessment and Planning: Conduct thorough assessments of our residents' health and develop individualized plans of care to meet their unique needs.
- Collaboration and Communication: Work closely with our interdisciplinary team to ensure seamless care coordination and effective communication.
Requirements:
- Experience: 1-3 years of experience in a long-term care setting, preferably in a leadership role.
- Licensure: Current Registered Nurse (RN) licensure in the state.
- Certification: CPR certification.
